Animal Health Certificate Form

Please note: if travelling with the owner, the named owner on the AHC paperwork must match the owner travelling both directions with the pet. The appointment to sign the paperwork must be completed by the named owner or the person authorized by the owner to travel with the pet. If the pet is travelling with an authorized person, we advise they carry a letter of authorization signed by the owner giving permission to carry the animal.
